Student Athlete Requirements and FAQ

All students who are joining San Bernardino Valley College intercollegiate teams must complete the following requirements before they will be eligible to participate in any sports activities.
 
1. Athletic Trainer System (ATS)
  • Once coaches submit team rosters to the Sports Medicine Staff, Athletes will need to access the ATS website and sign in to fill out all general information required for the Sports Medicine Department. Athletes required information needs to be received by the Sports Medicine staff before access to ATS can occur. Once access is available, the Sports Medicine staff asks athletes to follow these directions: How-To PowerPoint
2. Required Paperwork
 
  • All student athletes will be required to electronically fill out Pre-Participation paperwork online. All Pre-participation paperwork can be found on ATS. All forms are fill in and allow for electronic signature, making it easy and legible for staff. Please be sure to upload all paperwork on to ATS. Sports Medicine staff will confirm your paperwork was received.
Helmet/Mouthpiece Consent Form (Football Only)
  • All football athletes are required to fill out and upload on ATS a Football Helmet/Mouthpiece Consent Form in addition to the Pre-Participation Paperwork. Your Consent Form should be uploaded with your Pre-Participation Paperwork on ATS.
Physical
  • All student athletes are required to have a new sports physical completed every year before participation in athletic activities. The physical will need to be turned into the Sports Medicine Department before any participation in sport activity is allowed. No Physicals will be collected or accepted via fax, email, or handed in to a coach. All physicals will need to be uploaded and handed in person to the Athletic Trainer.
  • If you have had a surgical procedure or were medically instructed not to participate due to injury in the previous 12 months, we require that you provide the Sports Medicine Department with a clearance note from the physician who gave orders for no activity. You will need to provide a medical note stating that you are cleared to return to full sport activity (return to Physical Activity or Physical Education will not be accepted, it must state Sports Activity or similar). Without this note your clearance for practice and competition will be delayed.
3. SWAY BASELINE CONCUSSION TESTING:
  • Athletes are required to complete a SWAY baseline test before being cleared for participation. SWAY is a mobile device based neurocognitive and balance testing tool used in the management of mild traumatic brain injuries (mTBI), more commonly known as concussions. A staff Certified Athletic Trainer will provide an access code along with instructions to all athletes upon return to on-campus activities. Not following instructions properly can alter test scores. If test scores are unacceptable, you will be asked to re-take the exam before participation/clearance is approved.
 

Sports Physical FAQ

Where can I get my sports physical done?
 
We recommend you make an appointment with your primary care provider for a sports physical if possible. If one cannot be made with your primary provider, please feel free to utilize one of the following Urgent Care locations for a sports physical.
 
  • Colton Urgent Care 1181 N. Mt. Vernon Ave. Colton, CA 92324 – Open 9 a.m.-9 p.m. 7 days a week. Walk ins are accepted but would recommend making an appointment by calling 909-639-8800. Cost is $25 for a sports physical.
  • Loma Linda University Health Urgent Care 25828 Redlands Blvd, Suite 103. Redlands, CA 92373 – Open Monday – Friday 8 a.m.-7 p.m., Saturday, Sunday, Holidays 8am-3pm. Walk-ins are welcome. Cost is $25 for a sports physical.
Currently, the Athletics Departments DOES NOT intend to offer sports physicals on campus.
 
What do I need to take to my sports physical?
 
You need to take all sports physical documents to the doctor including the health history record. Please have the medical professional sign the sports physical upon completion and ask them to mark a designating status for participation to be medically cleared for participation.
 
Can any Doctor complete my sports physical?
 
No, sports physicals can only be completed by a Medical Doctor (MD) or Doctor of Osteopathic (DO). Any sports physicals completed by a Nurse Practitioner (NP) or Physician Assistant (PA) will need to also have their accompanying MD/DO stamp to be accepted. Sports physicals completed by Physical Therapist (PT) or Doctor of Chiropractic (DC) will not be accepted due to CCCAA bylaws .
 
Do I need all forms in the physical paperwork?
 
Yes, all forms in the physical paperwork including the Health History Record are required and need to be completed for you to be cleared for athletic participation.
 
When are my physical and Pre-Participation Paperwork information due?
 
All athlete’s physicals, and pre-participation paperwork requirements are due on their 1st report date for on-campus activities. Athletes will not be allowed to participate in any in-season athletic activity until all required paperwork is completed.
 

Athletic Trainer System (ATS) FAQ

What is Athletic Trainer System (ATS)?
 
Athletic Trainer System (ATS) is an online secure database helping the Sports Medicine Staff record, manage, and report athlete information. SBVC uses this program to maintain and track all student athlete medical and insurance information.
 
How do I get in the ATS system?
 
Ask your head coach to access the basic information excel sheet. He/She will have you fill out and/or ask the necessary information and email it to us. We will import you into the system, then, you will have access.
 
Why can’t I login into ATS?
 
First, check to make sure your login credentials are correct (username and password). Second, be sure the database is set to atsvalleycollege. Lastly, if you did not fill out or provide your basic information to your head coach on the excel prior to trying to login, you’re not in the system.
 
What do I do when I login?
 
Please carefully follow each step of the How-To PowerPoint linked below. This will ensure you fill out all necessary forms, fields, and fast track your ability to play.
 
Can I change my password?
 
Yes, but if you do so, please choose something easy for you to remember.
 
I turn 18 during season, do I need my parents’ signature?
 
Yes, all student-athletes 17 or younger at the time of starting with SBVC athletics are required to have a parent signature on all forms asking for one. Without it, you will not be cleared to practice or compete.
 
I can’t sign my documents. What do I do?
 
Be sure to download Adobe Fill & Sign. This app will allow you to sign your documents.
 
Can I sign up with any email address?
 
Yes, you can sign up with any email address however we highly recommend student athletes use their SBVC email. Please make sure the email provided is one used on a regular basis. Also be sure to check your junk/spam folder for emails that are coming directly from ATS.
